Kindergarten Health Assesment
The North Carolina Kindergarten Health Assessment (KHA) form is mailed
annually to the central office of each LEA (Local Education Agency) and to
the central office of each LHD (Local Health Department) in the early spring.
Other schools (charter schools, private schools) and Health Care Providers
can obtain the form by downloading the pdf file (see link below).
School systems and health care providers should discard (recycle) unused
forms from previous years that may be in storage, as the only acceptable
form for North Carolina kindergarten entry is the form dated 2008 (or
facsimile of it).
All components of the exam form are to be completed
by the health care provider except the sections reserved for parents
to complete.
